Camp John Hay Golf Club, Inc. For purposes of applying for the transfer of membership certificates to the name of the transferee, the Club requires the parties to execute a Data Privacy Consent Form, to authorize the Club, its stock and transfer agent, BDO Unibank, Inc., and the Club’s legal counsel, Poblador Bautista & Reyes Law Offices to process, collect, use, store, share, or disclose personal data which includes, name, address, gender, age, marital status, contact details, birthday, SSS/GSIS, TIN, education, employment information, financial information, spouse details, and other information classified as “personal information,” or “sensitive personal information” under the DPA between or among them as may be necessary for the transfer of membership in the Club. We will exert reasonable efforts to protect against unauthorized use or disclosure of the parties’ personal data. Parties should ensure that personal data submitted to the Club is complete, accurate, true, and correct.
